The Continuing Care Teams (CCT) provide community-based assessment, treatment, care and support for people with severe mental illness. The team’s partner with consumers, their families, and carers to identify and address their needs and assist in recovery through:
- Psychiatric, psychological and social, assessment and treatment
- Care Coordination with your family, GP, Private Psychiatrist, and support service providers
- Advocacy
- Crisis response including referral to more intensive support where required
- Liaison and planning to establish post-discharge supports and minimise the risk of relapse.
- Support periods generally vary between 3-12 months depending on your needs and engagement.
Our services operate out of four main sites:
- Doncaster and Koonung CCTs – 43 Carrington Road, Box Hill
