The INREACH team formed the Falls Outreach Risk Mobile Assessment Team (FORMAT) aiming to assess residents of Aged Care homes who have fallen on-site, bringing the emergency room to the most vulnerable people in our community.
During the FORMAT pilot period, the emergency department saw a 28% reduction in residents presenting after they had experienced a fall.
